Medical Massage and Insurance Billing
I am a preferred provider for massage therapy services for the following healthcare plans:

All treatments must be medically necessary as required by your healthcare plan. All medical massage claims therefore require a current prescription or treatment plan from your Primary Care Provider (PCP); that is, your Family Doctor, Chiropractor, Naturopath, etc. Please keep in mind that many plans do not require a referral, however a prescription, as written by your doctor is still required.
ALL prescriptions for massage therapy requires the following information:
- diagnosis codes (ICD-9 code)
- frequency of visits
- number of viists
- start date
- end date
Upon verification of insurance coverage for massage therapy and receipt of your prescription, I offer direct billing to your insurance provider. Deductibles and copays are required at the time of service unless prior arrangements have been made.
Many health care plans can have different rules regarding alternative therapy coverage. To fully understand your options and avoid any confusion, it is strongly recommended to contact your healthcare plan regarding your massage therapy benefits. I will be happy to offer any assistance possible and answer any questions.
Scheduling Appointments
All sessions are by appointment only and can be arranged by calling 206-579-0051, by email at kirasiplerlmp@hotmail.com or submitting an appointment request (See 'Request a Session'). I am happy to provide reminder calls at your request.
Your First Appointment
For your first treatment, please arrive 10-15 minutes early to complete any necessary paperwork.
For insurance treatments, please bring a copy of your insurance card and doctor's prescription/treatment plan. To expedite the intake process, you may complete the necessary forms (See 'Helpful Forms') and bring them to your first session.
Changes/Cancellations
Your session time has been reserved for you so if any changes are necessary, please provide 24 hours advance notice. Any late cancellations or no show appointments are charged the full amount of the scheduled session time. Insurance company may not be charged for your missed appointment; you will be responsible for payment out of pocket at the allowed amount charged predetermined by your health care plan.

My office is located in the University/Ravenna District at 5649 University Way, NE, Seattle, WA 98105. There is ample 2 hour parking and multiple bus lines that service the area. Look for the purple door!
